Survey Creation Tips

Keeping the number of questions small within your surveys is important. Most people will leave a survey before completion if it is too long or too tedious. An ideal survey size should be no longer than ten questions, with the last question remaining open-ended for additional comments or suggestions. These queries should focus clearly on the care a patient has received, with statements specific to quality of service and a controlled number of responses.

Listing important questions first maintains a logical order of the survey. Surveys should be conducted in a simple language that patients can easily understand and pertain to them. Reliable feedback is obtained when clients are asked questions objectively about all aspects of care.

In creating your survey answers, simple formats with either single response or multiple choices are most popular. Ranking is also an important rating of service with not much effort on behalf of the person being surveyed. There should be at least one open-ended text box for additional comments or concerns.

Privacy Concerns

Patients have the opportunity to either submit surveys anonymously or with contact information included. Anonymity promotes honesty and the ability to express the patients' exact thoughts clearly in several different categories. In this case, patients need not fear repercussions of his or her answers. Reporting is never released to the public, so required improvements in staff or service is internally guided.
