Online Patient Survey

Each patient is provided a secure log in to access the online surveys. This method protects privacy and prevents other users from compromising data. We provide a complete list of user names and passwords so each patient can access the particular survey to which you request.

The login information is supplied to the patients in a few different means. A patient can opt to be contacted directly by the healthcare professional or administrator via email. With an email address obtained with the initial patient documentation and paperwork, this approach is done efficiently as a follow-up to appointments. Using email access, healthcare professionals have the added benefit of cutting cost and amount of waste that typically exists with paper and printing supplies.

Phone Survey Dial-in

Each user is provided a 1-800 number to access the survey. This method protects privacy and promotes a personal touch to the process. We provide a list of unique IDs to protect the exactness of the data.

Talk to a Live Person

Patients answer survey questions by pushing buttons on a telephone keypad or by talking directly to a representative. We employ an experienced staff, prepared to complete the questionnaires with customers. Our personnel remain independent, and are prepared to explain any questions. This method may clear up any confusion or difficulty users may encounter in answering.
